Job summary

Team Select Home Care of Indiana, LLC seeks a compassionate Certified Nursing Assistant (CNA) to provide in-home personal care and ADLs under RN supervision. Follow the care plan, assist with daily living activities, document visits, and ensure infection control.

Qualifications

• High School Diploma or GED required.
• Must be 18 years of age or older per state regulations.
• CPR certification and health certifications per state requirements.
• Maintains licensure or certification per state regulation.

Responsibilities

• Provides personal care, household chores, meals, and other tasks per care plan.
• Assists with simple procedures as delegated.
• Assists with ambulation or exercises as assigned.
• Assists with medications ordinarily self-administered per state regulation.
• Maintains infection control practices and documents visits timely.
• Reports changes in patient condition to RN and follows agency policy.
