Team Select Home Care is seeking a Certified Nursing Assistant (CNA) to deliver skilled personal care and ADLs in patients' homes under RN supervision in Indiana.

• Maintains infection control practices and RN-directed policies.
• Completes visit records in a timely manner per policy.
• Reports changes in patient condition to RN.
• Attends staff meetings and care conferences.
• Maintains confidentiality of patient information.
