Job summary

Team Select Home Care of Indiana, LLC is seeking a Certified Nursing Assistant (CNA) to provide skilled personal care and activities of daily living to clients in their homes under the supervision of a registered nurse. You will report to the RN and deliver compassionate, patient-centered care.

The position emphasizes adherence to state regulations, infection control, timely documentation, and collaboration with the care team to support safety and quality home health services.

Responsibilities

• Provide direct patient care, ADLs, meals, and light housework as allowed.

• Support RN delegation tasks and report changes in patient condition.
• Maintain infection control practices and timely visit records.
• Attend staff meetings and care conferences as scheduled.
• Document care and ensure confidentiality of patient information.
