Job summary

Rocky Mountain Care-Clearfield is seeking a Certified Nursing Assistant to join our Clinical team in Clearfield, UT. This full-time, day-shift role provides CNA services per care plans under supervision, and supports residents with daily living activities in a long-term care setting.

The position requires CPR/BLS certification, a Food Handler Permit, and high school diploma or equivalent, with preferred experience in skilled nursing. Training and compliance are emphasized.

Qualifications

• Certified Nursing Assistant in good standing with the state (preferred, not required).
• High school diploma or equivalent.
• Current CPR/BLS certification
• Current Food Handler Permit
• Must have no disciplinary action in effect against certification.
• At least one year of experience as a CNA in long-term care (preferred, not required).

Responsibilities

• Provides supportive services to nurse, med tech and other staff as needed and performs duties as assigned.
• Assists resident with activities of daily living per care plans and established policies and procedures.
• Assists resident with lifting, turning, moving, positioning, and transporting.
• Coordinates dining room services during meals, including set-up, delivery, feeding assistance, and documentation of intake.
• Delivers nutritional supplements and documents intake accordingly.
• Assists with weighing residents and reports changes to nurse and supervisor.
• Monitors resident skin condition and reports issues.
• Assists nursing staff with toileting program activities.
• Completes flow sheets daily and documents progress.
• Possesses ability to lift at least 35 pounds and work 6a-6p as needed.
