Job summary

Qualifications

• At least one year of CNA experience in long-term care preferred.

Responsibilities

• Assist residents with activities of daily living per care plans.
• Assist with lifting, turning, positioning, and transferring residents.
• Coordinate dining room services, meal setup, delivery, feeding, and intake documentation.
• Deliver nutritional supplements and ensure intake is recorded.
• Weigh residents and report weight changes to nurse/supervisor.
• Monitor skin condition and report issues to nurse/supervisor.
• Assist with toileting programs and document progress.
• Complete flow sheets; document resident progress as directed.
• Communicate changes in residents' condition to nurse/supervisor.
• Maintain safety, hygiene, and confidentiality per facility policy.
