Job summary

Amedisys is seeking a dedicated hospice aide to support patients in home settings across Sussex, DE. This full-time role emphasizes compassionate care and adherence to the plan of care, with flexibility for day shifts and a focus on helping patients heal where they feel most comfortable—at home.

Experience in home health or hospice within the last 12 months is preferred. Must hold a DE CNA license and CPR certification; valid driver's license and reliable transportation are required.

Qualifications

• Current state certification as a nurse aide (CNA) per state requirements.
• CPR certification is current.
• A valid driver's license and reliable transportation.

Responsibilities

• Provide personal care services to patients in home/hospice settings.
• Apply safety principles and proper body mechanics during transfers, ambulation, and positioning.
• Plan and prepare nutritious meals and assist with feeding as needed.
• Perform homemaking and environmental services per care plan.
• Observe, report, and document changes in patient status; alert nurse as required.
• Follow written care instructions from the registered nurse before delivering services.
• Assist patients with prescribed exercises as directed by nurse/therapist.
• Perform and record simple urine tests for sugar, acetone, and albumin.
• Undertake other duties as assigned.
