Job summary

Amedisys is seeking a hospice aide to provide compassionate personal care to patients at home. You will follow plans of care, assist with daily living activities, and support family members in caregiving tasks.

The role involves mobility assistance, meal support, light homemaking, and documenting patient status for the nursing team. A valid CNA certification and CPR are required; a valid driver's license and reliable transportation are needed for home visits.

Qualifications

• High school diploma or equivalent.
• Current state CNA certification.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics for ambulation, transfer, ROM, and positioning, and assists with household chores.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents changes in patient's status and reports to the primary nurse if unable to perform tasks.
• Follows written instructions for personal care as prepared by a registered nurse.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

Education

High school diploma or equivalent
