Job summary

Presbyterian Healthcare Services is seeking a compassionate Certified Nursing Assistant to provide patient-centered hospice care in the home setting. You will work under the RN director, manage caseloads, and ensure timely delivery of services while supporting daily living activities and comfort measures.

The role includes documentation in EPIC and collaboration with the interdisciplinary team. Qualifications include CNA certification, BLS, and a valid driver’s license; prior home health

Qualifications

• High School diploma or equivalent.
• 1 year of direct patient care in any clinical setting preferred.
• CNA certification and BLS required.
• Proficient with EPIC and EMR systems.

Responsibilities

• Provide patient-centered care under RN direction in home hospice setting.
• Plan and navigate routes to patient service addresses.
• Maintain patient caseload and scheduling to ensure timely care delivery.
• Record vital signs and patient data in the care plan and EMR.
• Assist with activities of daily living and personal care.
• Support with wound care, meds prompts, and mobility assistance.
• Document care per agency policy and participate in IDT conferences.
