Job summary

Heritage-Woods-of-Mt.-Vernon in Mount Vernon, IL is seeking a Full-Time CNA for 2nd shift to deliver individualized resident care according to each resident’s care plan. You will assist with Activities of Daily Living while promoting independence and dignity.

Qualifications

• High School graduate or GED equivalent.
• Current CNA certification from an accredited program.
• Exceptional communication and interpersonal skills.

Responsibilities

• Deliver individualized care per resident care plans.
• Assist with Activities of Daily Living while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Engage with residents and families with compassion and impact.
• Participate in educational seminars and staff meetings.
• Maintain confidentiality and privacy of resident information.
• Perform other duties as assigned.
