Job summary

Heritage-Woods-of-Minook in Minooka, IL is seeking a compassionate CNA for a 2nd shift, part-time position. You will assist residents with daily living activities, follow care plans, and report changes to the clinical team while maintaining dignity and privacy.

Qualifications

• Current CNA certification from an accredited program.

Responsibilities

• Deliver exceptional and individualized care as determined by each resident’s care plan.
• Assist with Activities of Daily Living (ADLs) while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate and impactful manner.
• Engage in learning through seminars, staff meetings, and other learning experiences.
• Maintain confidentiality and privacy of resident information.
• Perform other duties as assigned.
