Job summary

Heritage Woods of Sullivan is seeking a part-time CNA to provide compassionate resident care in Sullivan, IL. You will assist with daily living activities, monitor health, and report changes to the clinical team, maintaining dignity and respect throughout all interactions.

This role features a 2nd shift schedule, opportunities for professional growth, and a benefits package designed to support residents and their families as outlined in the care plans.

Qualifications

• High School graduate or GED equivalent required.
• Current certification from an accredited Certified Nursing Assistant (CNA) program.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver exceptional and individualized care as determined by each resident’s care plan.
• Provide assistance with Activities of Daily Living (ADLs) while promoting independence and maintaining dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience.
• Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences.
• Comply with all rules and regulations regarding confidentiality and privacy of resident information.
• Perform any other duties as assigned.
