Job summary

Together Homecare of Fort Wayne LLC is seeking a Certified Nursing Assistant (CNA) to provide skilled personal care and ADLs in patients' homes under RN supervision. You will report to the RN and follow the approved care plan while documenting visits accurately.

The role requires compassionate care, ability to communicate effectively, and adherence to infection control practices. Competitive pay range is offered with benefits and ongoing training programs.

Qualifications

• High School Diploma or GED required.
• 18 years of age or per state regulation.
• Maintains licensure or certification per state regulation.
• Maintains current CPR certification and health certifications per state requirements.

Responsibilities

• Provide personal care, ADLs, and household tasks as ordered in the care plan.
• Assist with simple procedures under RN supervision.
• Assist with ambulation/exercises as assigned.
• Assist with medications ordinarily self-administered as directed.
• Maintain infection control practices and adhere to agency policies.
