Job summary

StoneBridge Senior Living seeks a compassionate Certified Nursing Assistant to support residents in a long-term care setting. You will work with medical staff to provide daily living assistance, monitor health changes, and document care activities with accuracy.

The role emphasizes empathetic patient support, teamwork with nursing staff, and adherence to care plans to enhance residents' quality of life. Training and ongoing development are encouraged.

Qualifications

• Must be a Certified Nursing Aide in good standing or graduate of a state-approved CNA program and pass a state-approved competency exam within 120 days of employment.
• High School Diploma or GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of experience as a CNA in long-term care preferred.
• Excellent written and oral communication in an interdisciplinary care setting.

Responsibilities

• Provide supportive services to nurses and other staff as needed and perform duties as assigned.
• Assist resident with activities of daily living per care plans and policies.
• Assist with lifting, turning, moving, positioning, and transferring residents.
• Coordinate dining room services at mealtimes, including setup, delivery, and feeding assistance; document meal intake.
• Deliver nutritional supplements and document intake as required.
• Weigh residents and report changes to nurse; document in designated locations.
• Monitor skin condition and report any issues to nurse and supervisor.
• Assist with toileting program activities.
• Complete flow sheets daily and document resident progress as directed.
• Inform nurse/supervisor of factors interfering with duties.
