Job summary

CommWell Health is seeking a dedicated healthcare professional to join our rural primary care team. You will deliver compassionate care across the lifespan, collaborate with providers, and support families in understanding health needs and next steps.

You will document care in our EHR, participate in care huddles, and contribute to team-based quality improvement initiatives within our Patient-Centered Health Home model.

Qualifications

• High School Diploma or GED required.
• Active CPR/BLS required.
• CMA Certification required.
• NC Driver’s license required.

Responsibilities

• Deliver compassionate, high-quality care to patients across the lifespan.
• Partner with providers and care teams to ensure seamless, coordinated treatment.
• Support patients and families in understanding health needs and next steps.
• Document care accurately using our EHR system.
• Participate in care huddles and team-based quality improvement initiatives.
• Serve as a key member of our Patient-Centered Health Home model.
