Job summary

CommWell Health in rural North Carolina is seeking a compassionate nurse to join our patient-centered care team. You will deliver high-quality care across the lifespan and partner with clinicians to coordinate treatment close to home.

This role emphasizes education for patients and families, precise documentation in our EHR, and participation in care huddles and quality initiatives within our Health Home model.

Qualifications

• High School Diploma or GED required.
• CPR certification required.
• CMA Certification or LPN license required.
• Valid NC Driver’s license required.

What We’re Looking For

Required:

• High School Diploma or GED
• CPR
• Valid CMA Certification/ Valid LPN license
• Valid NC Driver’s license

Preferred:

• One (1) year of clinical experience in a primary care, community health, or outpatient setting
• Experience with underserved or rural populations
• Familiarity with team-based care or PCMH/PCHH models
