Job summary

SYNERGY HomeCare is seeking a Case Manager to coordinate caregiver scheduling, maintain strong client relationships, and ensure efficient operations. You will work with clients, caregivers, and providers to deliver reliable home care services.

The role requires strong organizational and communication skills, a proactive mindset, and the ability to work independently in a fast-paced environment. CPR/First Aid and Level 1 FCC are required within 30 days of hire.

Qualifications

• Experience – One year in home care preferred; knowledge of ALTCS/DDD contracts a plus.
• Education – Associate degree or 5 years in a customer service setting.
• Skills – Exceptional customer service, time management, and communication.
• Characteristics – Autonomous, self-motivated, eager to learn.
• Requirements – CPR, First Aid, and Level 1 FCC within 30 days of hire.

Responsibilities

• Schedule caregivers with clients and communicate with all parties.
• Utilize scheduling software to enhance productivity.
• Maintain high-level communication with colleagues.
• Be a team player and collaborate effectively.
• Conduct daily/weekly reporting and quality calls/visits.
• Update caregiver availability, payroll, and contract provider relationships.
• Document caregiver skills for strong client matching.
• Onboard and orient new hires; train and mentor caregivers.
