Job summary

Hamilton Center, Inc. is seeking a Care Manager I / CCBHC Care Coordinator II to provide case coordination, outreach, and advocacy for individuals entering CCBHC. The role includes initial screening, referrals, and development of treatment plans with psychosocial evaluation when applicable.

The position requires a background in behavioral health care, strong communication, and the ability to work with diverse populations. Experience with EMR systems and a valid driver’s license is desired.

Qualifications

• High School diploma or equivalent.
• 1 year of paid, full-time employment or 24 months of paid, part-time employment in a related adult, child, youth, adolescent or family services field.
• For CCBHC Care Coordinator II: Bachelor's degree in psychology, social work, sociology or related fields with 1 year of case management experience; or non-specific bachelor's with 4+ years in related field.
• Completion of Case Management training post college degree.
• Knowledge of community-based behavioral health care and case management.
• Willingness to use reliable personal transportation in work capacity.

Responsibilities

• Consistently exercising discretion and judgment to analyze, interpret, and decide actions based on facts.
• Work without daily supervision, evaluating possible courses of conduct.
• Maintain an active caseload providing referrals and linkage within areas of expertise.
• Provide team with detailed information to establish Treatment Plans.
• Work with the consumer daily to implement the Treatment Plan.
• Develop and revise Treatment Plans and monitor goals.
• Communicate with community partners to maintain MOUs.
• Coordinate with community systems for linkage, crisis management, and follow up.
• Provide crisis management and linkages for interventions as needed.
• Provide psychosocial rehabilitation services to consumers.
• Deliver face-to-face skill training and mental health interventions.
• Offer age-appropriate life skills training.
• Maintain records per standards and regulations.
• Demonstrate ethical conduct and confidentiality; use EMR competently.
• Participate in continuing education and supervision meetings.
• Complete required trainings (CANS/ANSA) and DSC paperwork timely.
• Participate in clinical supervision as needed.
• Maintain flexible scheduling with supervisor.
