Qualifications

Responsibilities

• Manage a designated caseload to coordinate discharge planning and care transitions.
• Communicate with care teams about assessment findings and discharge needs.
• Coordinate arrangements for Homecare, DME, transportation, and post-acute placements.
• Ensure compliance with CMS and related regulations and educate patients and families.
• Collaborate with Utilization Management to support payer negotiations and prevent denials.
• Complete initial psychosocial assessment within 72 hours.
• Engage patient support systems and family in discharge planning.
