Qualifications

• BA/BS in Social Work or related field or 2 years of mental health experience.
• Strong communication, organization, and problem-solving abilities.
• Ability to coordinate multiple activities and meet deadlines.
• Basic computer skills and First Aid certification.

Responsibilities

• Provide case management and support to residents.
• Assist residents with daily living skills and managing psychiatric symptoms.
• Coordinate medical, social, educational, and rehabilitative services.
• Develop and implement individualized service plans.
• Collaborate with residents, families, staff, and outside providers.
• Monitor resident progress and document service outcomes.
• Prepare annual assessments, service plans and progress notes.
• Provide support during potential crises and develop appropriate interventions.
• Transport residents to appointments and outings as needed.
• Assist with group treatment and recovery-oriented activities.
