Creekside Post-Acute is seeking an Admissions Coordinator to manage the admission process for new residents in skilled nursing and post-acute care. You will coordinate with hospitals, families, and referrals, verify demographics and insurance, and maintain complete records with confidentiality.

The ideal candidate will be highly organized, professional, and capable of handling sensitive information while delivering outstanding customer service in a fast-paced healthcare setting.

Qualifications

• Experience in Admissions, Intake, Patient Services, or related healthcare role preferred.
• Experience in Skilled Nursing or Post-Acute Care preferred.
• Strong communication and organizational skills with attention to detail.

Responsibilities

• Coordinate admission process from referral through admission.
• Verify patient demographics, medical, and insurance information.
• Verify insurance eligibility, benefits, and coverage requirements.
• Communicate with hospitals, physicians, referrals, residents, and families.
• Maintain accurate admission records and documentation.
• Ensure paperwork is obtained before and during admission.
