Job summary

SEARHC is seeking a Community Wellness Coordinator within the Nutrition Services and Women’s Health programs in Southeast Alaska. You will coordinate care, assess patient self-management, communicate with patients, and manage referrals to ensure timely, high-quality outcomes.

You’ll support scheduling, data collection, and cross-team collaboration while promoting excellent customer service and adherence to safety and compliance standards.

Qualifications

• Associate’s degree or 2 years of college in social work, health admin, or related field.
• Bachelor’s degree may be exchanged for experience.
• 2 years healthcare/administrative experience required.
• Basic Life Support preferred.

Responsibilities

• Assists the Community Wellness Division in coordinating safe, effective, efficient, equitable, patient-centered care.
• Assesses the self-management skills of patients and caregivers and encourages wellness and autonomy.
• Communicates regularly with patients and caregivers regarding referred services.
• Coordinates care coordination within and outside of the SEARHC health system to ensure a consistent system of care.
• Tracks and maintains case management coordination data identified by program managers.
• Schedules patient appointments and manages WellApp communications.
• Maintains data input in designated database for consortium-wide access.
