Job summary

Total Life is seeking a Longevity Ambassador to represent Total Life across Southern California, covering communities in Fullerton, Buena Park, and surrounding areas. In this role you will conduct in-person wellness assessments with older adults, build relationships with physician practices and residential sites, and act as the friendly local contact for clients, caregivers, and partners.

You will host longevity screening events, log outreach in our CRM, and collaborate with our virtual therapy

Qualifications

• Extroverted and cheerful; enjoys starting conversations with people
• Comfortable visiting physician offices or residential communities without an appointment
• Warm and genuinely interested in older adults
• Healthcare background or actively enrolled healthcare student (MA, CNA, nursing, social work, public health, gerontology)
• Available a minimum of 10 hours per week
• Reliable transportation for local travel
• Comfortable using a CRM and standard phone and email tools
• Able to transport and set up booth materials
• Must be able to pass a criminal background check

Responsibilities

• Conduct in-person wellness assessments for older adults to evaluate emotional, cognitive, and social well-being
• Represent Total Life in the community and residential partner sites as the local point of contact for clients, caregivers, and partners
• Build and maintain referral relationships with physician practices, senior living communities, and home health agencies across your territory
• Host Total Life’s longevity screening events at partner sites and assist attendees in booking their first appointment
• Collaborate with the virtual therapy team to ensure smooth transitions into ongoing care
• Maintain accurate documentation and log outreach activity in our CRM daily
• Participate in team meetings and contribute to improving community-based programs
