Job summary

Total Life in Florida is seeking a Longevity Ambassador to represent our mission in the community, conducting in-person wellness assessments for older adults and serving as the local point of contact at partner sites.

You will build relationships with physician practices and residential communities, host longevity screening events, and coordinate with the virtual therapy team to ensure smooth transitions into ongoing care.

Qualifications

• Extroverted and cheerful; enjoys starting conversations with people.
• Comfortable visiting physician offices or residential communities without an appointment.
• Healthcare background or enrolled healthcare student (MA, CNA, nursing, social work, public health, gerontology).
• Available a minimum of 10 hours per week with reliable local transportation.
• Willing to pass criminal background check.

Responsibilities

• Conduct in-person wellness assessments for older adults to evaluate emotional, cognitive, and social well-being.
• Represent Total Life in the community and partner sites as the local point of contact for clients, caregivers, and partners.
• Build referral relationships with physician practices, senior living communities, and home health agencies across your territory.
• Host Total Life's longevity screening events at partner sites and help attendees book their first appointment.
• Collaborate with our virtual therapy team to ensure smooth transitions into ongoing care.
• Maintain accurate and timely documentation, and log all community outreach activity in our CRM the same day.
• Participate in team meetings and contribute to improving our community-based programs.
