Total Life is seeking a Longevity Ambassador to represent us across Greater Southern California in in-person settings. You’ll conduct wellness assessments for older adults, build relationships with physician practices and residential sites, and host screenings at partner locations.

You’ll log activities in the CRM and coordinate with virtual therapy for seamless care transitions. Ideal candidates are extroverted, healthcare-oriented, and able to commit at least 10 hours weekly with reliable

Qualifications

• Genuinely extroverted and cheerful; a large part of this role is starting conversations with people you have never met, and enjoying it.
• Healthcare background or currently enrolled in related field (e.g., Medical Assistant, CNA, nursing, social work, public health, gerontology).
• Available a minimum of 10 hours per week and reliable transportation for local travel.

Responsabilités

• Conduct in-person wellness assessments for older adults to evaluate emotional, cognitive, and social well-being.
• Represent Total Life in the community and residential partner sites as the local point of contact for clients, caregivers, and partners.
• Build and maintain referral relationships with physician practices, senior living communities, and home health agencies across your territory with in-person visits and follow-up.
• Host Total Life’s longevity screening events at partner sites and help attendees book appointments.
• Collaborate with our virtual therapy team to ensure smooth transitions into ongoing care.
• Maintain accurate and timely documentation in our CRM the same day.
