Elder Services of Cape Cod and the Islands seeks a Community Transition Liaison in South Dennis, MA. This full-time professional role serves as the onsite contact for residents, families, SNF staff, guiding transitions from skilled nursing facilities back to the community.

Responsabilités

• CTL acts as onsite point of contact for residents, families, SNF staff and others involved.
• Conduct visits with residents to increase awareness of services and transitions to the community.
• Support coordination and communication throughout transitions from NF to community.
