Job summary

Elder Services of Cape Cod and the Islands in South Dennis, MA is seeking a Family Caregiver Specialist to support caregiver programs and Medicare guidance. The role covers information requests, care navigation, and outreach, with a 34-hour workweek and hourly pay between $32.34 and $33.80.

Qualifications

Responsibilities

• Respond to information requests from prospective consumers and community agencies regarding caregiver programs.
• Assist consumers within the parameters of the MFCSP and Medicare’s GUIDE Program as a Care Navigator.
• Conduct telephone calls, in-home visits and telephone assessments; research resources and caregiver topics.
• Assist with caregiver support groups and workshops; manage documentation and participate in community outreach.
