Job summary

Elder Services of Cape Cod and the Islands is hiring a Family Caregiver Specialist in South Dennis, MA. This full-time role involves coordinating caregiver information, resources, and support services for the Elder Services community.

Qualifications

• Ability to collaborate, take initiative, provide/ receive feedback.
• Ability to communicate and work with the elderly and community resources serving the elderly population.
• Ability to work independently and professionally.
• Valid MA driver's license and car required.

Responsibilities

• Respond to requests for information from prospective consumers and community agencies regarding the Family Caregiver Support Program and Medicare’s GUIDE Program.
• Serve as a Care Navigator performing telephone calls, in-home visits, and assessments, researching resources and caregiver topics.
• Assist with caregiver support groups and workshops and manage computer documentation.
