Job summary

OCES is expanding to provide coordinated services for older adults and individuals with disabilities, including home visits and community outreach. The role focuses on assessing needs, developing individualized plans, and maintaining strong relationships with consumers and resources.

We seek compassionate, adaptable staff with excellent communication and computer skills, capable of working across our hybrid schedule, with opportunities for professional growth and a positive team environment.

Qualifications

• Minimum of one year of experience in the human services field.
• Experience in lieu of bachelor’s degree with ability to obtain a waiver from Executive Office of Aging Independence (AGE).
• Excellent written and verbal skills.
• Excellent computer skills.
• Proficient with Microsoft Office Suite or related software.

Responsibilities

• Conduct comprehensive needs assessments and develop individualized care plans
• Develop rapport with consumers to facilitate coordination, monitoring, and adjusting care plans as needed
• Provide regular contact with consumers, including home visits, to ensure progress toward established goals
• Contact and maintain communication with appropriate community resources to better coordinate delivery of consumer services
• Maintain a detailed knowledge of current OCES policies, and standards, funding source regulations
• Maintain accurate documentation

OCES is growing! We are expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, etc. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, safe living for our consumers, we hope to allow them to stay active and engaged in their lives and in their own communities for as long as possible.

What You'll Do
