Qualifications

High school diploma required.
Valid driver's license (USA) required.
Experience with individuals with mental health diagnosis or developmental disabilities or substance use disorder preferred.

Responsibilities

Assess and support people in pursuing hopes and goals.
Help develop relationships in the community and with natural supports.
Assist participation in the community aligned with interests.
Protect and promote rights; support advocacy efforts.
Provide emotional, physical, and medical support as needed.
Complete documentation including progress notes and incident reports.
Assist with daily living tasks and transportation as required.
Follow safety procedures and emergency procedures; participate in trainings.
