Service Coordination, Inc. is seeking Supports Planners to advocate for community-based services for individuals with disabilities. The role emphasizes solution-based planning and person-centered care, with training and employer-provided equipment.

Qualifications

• Bachelor's or Master's degree in a human service field.
• Experience with person centered planning preferred.

Responsibilities

• Advocates for people served, provides resource information and education.
• Plans for and updates Person Centered Plans of Service (POS).
• Develops POS tools and documents service needs for POS eligibility.
• Monitors POS and conducts follow-ups with providers.
