Job summary

Service Coordination, Inc. is seeking dedicated Service Coordinators to provide case management in the Gaithersburg, MD area. The role focuses on advocating for underserved individuals and coordinating community-based services.

Compensation includes a base salary within a defined range, travel reimbursement, quarterly bonus potential, and a comprehensive benefits package with tuition reimbursement and 403(b) matching. Training and equipment are provided to support success.

Qualifications

• A bachelor's degree from an accredited education program in a human services field; OR a bachelor's degree in a non-human services field, with two years of experience in a human services field.
• Experience working with people with developmental disabilities.

Responsibilities

• Advocate on behalf of individuals, provide resources, and educate them to become more self-determined.
• Collect information and documentation related to eligibility for DDA services and recommend eligibility and priority.
• Plan for, facilitate, develop, and continually update Person Centered Plans (PCPs).
• Research, identify, refer, and coordinate resources to achieve PCP outcomes.
• Monitor PCPs and conduct follow-up activities.
• Adhere to SCI policies and procedures.

Education

Bachelor's degree in a human services field; OR Bachelor's degree in a non-human services field with two years experience in human services
