State of Maryland seeks a Developmental Disabilities Targeted Case Management (TCM) Coordinator of Community Services (CCS) for the Dorchester County area. The role acts as a fixed entry point into the service delivery system, helping individuals prepare applications for DDA eligibility and facilitating PCP development and monitoring.

The position requires a relevant degree (Human Services) or equivalent experience, with additional qualifications noted.

Competenze

• Bachelor's degree in a Human Services field OR Associates degree with two years in a Human Services field OR seven years in a Human Services field.
• One year of administrative staff or professional work in a health care setting.
• Candidates may substitute 30 college credit hours for the required experience.

Mansioni

• Provide fixed entry into the service delivery system for individuals with developmental disabilities.
• Assist individuals in preparing applications and gathering documentation for DDA eligibility.
• Coordinate development and monitoring of the Person-Centered Plan (PCP) and related services.
• Complete additional assessments as determined by client situation.
