Job summary

Qualifications

• Bachelor’s degree, or Associate’s degree plus 3 years of professional experience, or 6 years of professional work experience.
• Excellent analytical and critical thinking skills; effective organizational and time management skills.
• Great attention to detail and follow up.
• Ability to manage projects, assignments, and competing priorities.
• Proficient in Zoom, Teams, and Microsoft Office, including Outlook, Word, and Excel.

Responsibilities

• Serves as a link between Parish Health Units and community programs and resources.
• Meets clients and conducts needs assessments, helps set goals and follows up with visits.
• Assists with applications for Medical Assistance and SNAP, and connects to resources.
• Documents client encounters and prepares reports for program supervisors.
• Attends meetings and participates in community engagement events across parishes.
