Job summary

LSU New Orleans is seeking a community program liaison housed at the St. Mary Parish Health Unit. The role meets clients, conducts needs assessments, and helps with Social determinants tasks such as housing, hunger, and employment.

Documentation and follow-ups are essential as is collaboration with BRCO and other team members. Responsibilities include scheduling visits, assisting with Medical Assistance and SNAP applications, and building a directory of local resources across parishes.

Qualifications

• Bachelor’s degree, or Associate’s degree plus 3 years of professional experience, or 6 years of professional work experience.
• Excellent analytical and critical thinking skills; effective organizational and time management skills.
• Proficient in the use of Zoom, Teams, and Microsoft Office, including but not limited to Outlook, Word, and Excel.

Responsibilities

• Acts as a link between Parish Health Units (PHUs) and community programs and resources for BRCO.
• Meets clients in regional PHUs or other community locations and conducts needs assessments.
• Assists clients with setting goals and following up with visits and calls.
• Helps clients with social issues such as homelessness, hunger, and employment.
• Aids clients with applications for Medical Assistance and SNAP.
• Develops and maintains a directory of community resources in the region.
• Attends community events and documents encounters in detail.
• Prepares reports and attends scheduled meetings.
