Job summary

Qualifications

• Bachelor’s degree or associate degree plus 3 years of professional experience.
• Excellent analytical and critical thinking skills.
• Great attention to detail and follow up.
• Ability to manage projects, assignments, and competing priorities.
• Proficient in Zoom, Teams, and Microsoft Office, including Outlook, Word, and Excel.

Responsibilities

• Acts as a link between the Parish Health Units and community programs for BRCO.
• Meets clients in regional PHUs or community locations to conduct needs assessments.
• Makes regular follow-up calls and in-person visits with clients.
• Motivates clients to meet identified goals.
• Assists clients with Medical Assistance and SNAP applications.
• Develops and maintains a directory of community resources in the region.
• Attends community events in parishes within the region.
• Documents each client encounter in detail.
• Prepares reports and documents for program supervisors.
• Attends scheduled program and regional meetings.
