University of New Orleans is seeking a community programs liaison based at the Sabine Parish Health Unit in Many, LA, serving Sabine, Natchitoches, Webster, Bienville, and Claiborne parishes. You will connect PHUs with resources, conduct needs assessments, and support clients to achieve goals.

Responsibilities include follow-up visits, assisting with Social Services applications (Medical Assistance and SNAP), and maintaining community resource directories.

Competenze

• Bachelor's degree or Associate's degree with 3+ years of experience; or 6 years of professional work experience.
• Excellent analytical and critical thinking; strong organizational and time-management skills.
• Great attention to detail and follow up.
• Ability to manage projects, assignments, and competing priorities.
• Proficient in Zoom, Teams, and Microsoft Office, including Outlook, Word, and Excel.

Mansioni

• Serves as a link between the Parish Health Units (PHUs) and community programs and resources for the BRCO.
• Meets clients in regional PHUs or other locations and conducts needs assessments, including helping patients set goals.
• Makes regular follow-up calls and in-person visits with clients.
• Motivates clients to meet their identified goals.
• Helps clients with social issues like homelessness, hunger and employment.
• Assists clients with making follow-up appointments and applying for Medical Assistance and SNAP.
• Works with the team to maintain a directory of community resources in region parishes.
• Attends community engagement events and regional meetings.
• Documents each client encounter in detail and prepares reports as needed.
• Other tasks as assigned.
