Job summary

Cantex is seeking a Community Relations Coordinator in Frisco, TX to drive census growth by coordinating referrals, tours, and admissions.

You will ensure accurate documentation and timely conversions, collaborating with clinical and business staff to meet occupancy targets and maintain resident safety.

Strong verbal and written communication, English proficiency, and experience in long-term care are preferred.

Qualifications

• Preferably one-year experience in a long-term care setting with demonstrated evidence of census development.
• Excellent verbal and written skills required; public speaking skills may be necessary.
• Ability to read, write, and speak English.

Responsibilities

• Obtains family commitment to tour Community on viable referrals and conducts a professional tour.
• Conducts the admission process, explains policies, and ensures paperwork is complete upon admission.
• Coordinates gaining admission approval for all referrals.
• Ensures that budgeted census mix is met or exceeded.
• Attends daily department manager meetings to update on referrals and admissions.
• Establishes a ninety-day marketing plan to achieve occupancy and quality targets.
• Identifies barriers to admissions and communicates them to leadership.
• Performs sales calls to targeted referral sources including physicians, hospitals, social workers, and community organizations.
• Documents inquiries using the inquiry follow-up log.
• Serves on committees as requested by the Executive Director.
• Adheres to Clinical Policies and Procedures for Abuse Prevention and Resident Care Management Systems.
• Attends weekly in-services and ensures patient/resident safety.
• Performs other duties as assigned.

Job Summary

The Community Relations Coordinator has responsibility for ensuring that all information is obtained, documented, and communicated to the appropriate staff within the Community. The Community Relations Coordinator is responsible for timely conversions of inquiries to admissions.

• Responsible for obtaining family commitment to tour Community on viable referrals. Conducts a professional tour utilizing clinical and financial personnel with objective of gaining admission.
• Conducts the admission process of signing and explaining admission policies and procedures to residents and family. Ensures all paperwork is complete upon admission.
• Coordinates gaining admission approval for all referrals.
• Ensures that budgeted census mix is met or exceeded.
• Attends daily department manager meeting to update on referrals, admissions and follow up on all outstanding issues to obtain admission.
• Responsible for ninety-day marketing plan to achieve goals for occupancy and quality mix targets.
• Identifies barriers to the admissions process and communicates barriers to the Executive Director and Director of Business Development.
• Responsible for conducting sales calls to designated targeted referral sources which may include but not be limited to physicians, hospitals, social workers, and community organizations.
• Documents all necessary information on inquiries utilizing inquiry follow-up log.
• Serves on various committees as requested by Executive Director.
• Has reviewed Cantex Continuing Care Networks Clinical Policies and Procedures for Abuse Prevention and knows the employee's responsibility to enforce it.
• Attends weekly in-services, understands, and complies with Resident Care Management Systems.
• Responsible for assuring patient/resident safety.
• Performs other duties and/or tasks as assigned.
