Job summary

Comfort Keepers in Petoskey, MI is hiring a Client Care & Community Relations Coordinator. You will manage client and family relationships, oversee care coordination, and grow community partnerships across northern Michigan.

The role requires travel across multiple counties, in-home visits, and collaboration with the Operations Team to ensure high-quality service and client retention.

Qualifications

• Associate degree or equivalent career experience preferred.
• Supervisory/leadership experience preferred.
• Valid driver’s license and ability to travel regionally.

Responsibilities

• Serve as a local point of contact for clients and families, building trust through regular communication and follow-through.
• Coordinate care team assignments, scheduling, and care plans with nursing and scheduling staff.
• Build relationships with hospitals, referral partners, and community organizations to support referrals and community outreach.
• Conduct in-home visits and consultations to understand client goals and service needs.
• Document all activity in the electronic records system and ensure timely updates.
• Address client and caregiver concerns promptly and coordinate next steps.
