Job summary

Weber County Sheriff's Office seeks a grant-funded Reentry Case Manager to provide targeted case management, resource education, and reentry planning for individuals released from the correctional facility, connecting them to housing, healthcare coverage, employment, and other supports to reduce recidivism.

Under supervision, you will coordinate services with treatment providers, social services, and community partners; maintain documentation, participate in team meetings, and stay current on

Qualifications

• Minimum of a high school diploma or GED; Bachelor's degree preferred in social services, corrections, addiction treatment, or related field.
• Knowledge of substance use disorders, mental health, and criminal justice system; working knowledge of community resources.
• Ability to work in a multi-disciplinary team and maintain confidentiality.

Responsibilities

• Conduct needs assessments and develop reentry plans with measurable outcomes.
• Coordinate access to MOUD/MAUD and other treatment services pre- and post-release.
• Identify community resources for housing, employment, education, healthcare coverage, and transportation.
• Maintain accurate case documentation and participate in cross-agency team meetings.
