Job summary

Connecticut Community Care is seeking a Program Navigator to assist in coordinating and implementing long-term services and support within the community. The role supports CCC’s mission by guiding clients through enrollment and activation across programs.

The candidate should have strong communication skills, experience in human services, and familiarity with Connexus. A valid driver’s license and reliable transportation are required, with a team-oriented, confidential approach to care.

Qualifications

  • High School Diploma required; Associate Degree in Health or Human Service preferred.
  • Two years’ experience in a human service setting for High School graduates; one year for Associate Degree holders.
  • Project management and organizational skills.
  • Strong verbal and written communication skills.
  • Knowledge of community resources preferred.
  • Ability to maintain confidentiality of client and company information.
  • Ability to understand and articulate multiple programs.
  • Working knowledge of Connexus.

Responsibilities

  • Provides referral screening for all new potential clients of Public Programs supported by CCC.
  • Provides telephonic communication with client and/or referral source to provide education and program navigation.
  • Schedules initial assessments.
  • Supports pending clients post-assessment to activate onto appropriate program(s); home visits as needed.
  • Works as part of an interdisciplinary team.
  • Mentors new staff as needed.

Summary

Under the direction of the Asst. Director the Program Navigator assists in the coordination and implementation of long-term services and support in the community. At CCC all duties are performed in a manner that fosters the achievement of the organization’s mission.

Essential Functions
  1. Provides Referral Screening for all new potential clients of Public Programs supported by CCC.
  2. Provides telephonic communication with client and/or the referral source to provide education and program navigation.
  3. Schedules initial assessments
  4. Follows, supports and assists pending client's post-assessment to assist with timely activation onto appropriate program/s. This will include home visits as needed.
  5. Participates in and works effectively as part of an interdisciplinary team.
  6. Assist in mentoring new staff as needed.

Requirements

Education:

High School Diploma required; Associate Degree in Health or Human Service preferred

Experience:
