Job summary

Family Centers in Connecticut is seeking a Community Resource Navigator to help individuals and families navigate healthcare and social service systems, develop individualized service plans, and connect clients with essential resources. If you are compassionate, resourceful, and culturally responsive, you will play a critical role in improving client outcomes and equitable access.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Public Health, Psychology, or a related field; or equivalent experience in case management or care coordination.
• Excellent interpersonal, written, and verbal communication skills.
• Strong organizational skills with the ability to manage multiple priorities.
• Valid Connecticut driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive needs assessments to identify health, social, financial, and community support needs.
• Partner with clients to develop individualized service plans that address their goals and challenges.
• Provide ongoing support, advocacy, and follow-up to help clients achieve positive outcomes.
• Assist clients with accessing and enrolling in public benefit programs such as SNAP, Medicaid, Medicare, SSI/SSDI.
• Collaborate with healthcare providers and community partners to coordinate care.
