Job summary

Central Coast VNA & Hospice is seeking a Community Specialist to provide administrative and operational support across programs, including Volunteer and Bereavement Services. You will assist with patient communication, scheduling, data tracking, and EMR documentation.

The role requires collaboration with internal teams, program awareness activities, and referrals management. A high school diploma and at least 1 year in healthcare are preferred; bilingual Spanish is a strong plus, with reliable

Qualifications

• High school diploma or GED required.
• At least 1 year of healthcare experience preferred.
• Strong interpersonal and communication skills.
• Bilingual in Spanish strongly preferred.
• Proficiency with Microsoft Office, especially Excel.

Responsibilities

• Provide administrative and operational support across programs, including referrals, admissions, scheduling, data tracking, and reporting.
• Coordinate and complete referral processes, obtain authorizations, verify eligibility, and communicate with clinical staff.
• Conduct bereavement follow-up calls and document outcomes.
• Support volunteer program activities and leadership assistance.
• Plan and execute events, including logistics and coordination.
• Maintain EMR documentation and internal databases with accuracy.
