Passavant Memorial Homes in Harmar is seeking a Community Participation Supports Director to lead CPS and SE services in the assigned region. The role reports to the Program Administrator and oversees CSPs and participants under PMH policies.

The CPS Director will coordinate daily schedules, ensure IP accuracy, supervise staff, conduct trainings, and maintain compliance with ODP service descriptions, while building community partnerships and advancing client outcomes.

Qualificações

• Must be at least 21 years old.
• Minimum two years of supervisory experience.
• Valid driver's license.
• Eligible to perform Program Specialist duties per 55 Pa. Code Chapter 6400.44.
• Ability to organize tasks and work independently.
• Master's degree or higher with related experience.
• Bachelor's degree with related experience.
• Associate degree / 60 credit hours with related experience.

Responsabilidades

• Oversee CPS and SE services, general management and policy adherence.
• Participate in client team meetings, IP reviews, and consultations.
• Ensure IP accuracy, revisions, and goal/objective monitoring.
• Coordinate participant schedules with residential managers and staff.
• Adjust weekly schedules reflecting changes and communicate updates.
• Train staff on IP, MAR, diet, needs, and treatment plans.
• Ensure timely Service Note completion for each participant.
• Complete Quarterly Progress Notes and Conversations.
• Perform Comprehensive Assessments annually.
• Maintain regional petty cash funds and reports.
• Oversee vehicle and equipment maintenance.
