Job summary

Passavant Memorial Homes is seeking a full-time Home and Community Services Program Coordinator to support the Program Director in daily operations across the Home and Community Services program.

The role covers traveling throughout Lawrence County with the corporate office in Harmarville, PA, and requires ensuring IPs are followed, coordinating with the Supports Coordinator, and training ISO workers while maintaining compliance with PMH policies.

Qualifications

• Master’s degree or higher from an accredited college or university and one (1) year experience working directly with individuals with developmental disabilities
• Bachelor’s degree from an accredited college or university and two (2) years’ experience working directly with individuals with developmental disabilities
• Associate’s degree, or 60 credit hours, from an accredited college or university and four (4) years’ experience working directly with individuals with developmental disabilities.
• Five (5) years’ experience working directly with individuals with developmental disabilities and strong demonstration of managerial and leadership skills.
• Valid PA Driver’s license.

Responsibilities

• Ensure IPs are implemented as written.
• Report any change related to the individual's needs to the Supports Coordinator.
• Recommend revisions to a service or outcome in the IP as needed.
• Ensure all ISO Support Service Workers are trained on the individuals IPs.
• Attend IP/Interdisciplinary Team meetings as needed.
• Interact with other departments to meet individuals needs.
• Input information for HCS ISO Support Service Worker pay.
• Ensure compliance with all applicable regulations and PMH policies and procedures.
• Provide emergency coverage as needed and directed.
• Participate in the on-call rotation for HCS.
