Job summary

Passavant Memorial Homes is seeking a full-time Home and Community Services Program Coordinator to support the Program Director in daily operations and coordination of services for individuals with developmental disabilities. The role covers travel throughout Lawrence County, with the home office at our Harmarville Corporate Office.

Responsibilities include IP implementation, team coordination, training, and maintaining accurate consumer management data while ensuring compliance with PMH

Qualifications

• Master’s degree or higher with 1 year experience working directly with individuals with developmental disabilities.
• Bachelor’s degree with 2 years’ experience working directly with individuals with developmental disabilities.
• Associate’s degree, or 60 credit hours, with 4 years’ experience working directly with individuals with developmental disabilities.
• Five (5) years’ experience working directly with individuals with developmental disabilities and strong leadership skills.
• Valid PA Driver’s license.

Responsibilities

• Ensure IPs are implemented as written.
• Report changes to supports coordinators.
• Recommend revisions to services/outcomes in IPs.
• Ensure ISO Support Service Workers are trained on IPs.
• Attend IP/Interdisciplinary Team meetings as needed.
• Interact with other departments to meet individuals’ needs.
• Enter information in Consumer Management accurately.
• Input information for HCS ISO Support Service Worker pay.
• Ensure compliance with regulations and PMH policies.
• Support in engaging potential HCS ISO workers.
• Complete ISO worker startup requirements.
• Provide emergency coverage as needed and be on-call for HCS.
• Communicate effectively with all levels and external partners.
• Perform other duties as assigned by the HCS Director.
