Job summary

Qualifications

• Excellent verbal and written communication with guests, staff, volunteers.
• Ability to work independently and as part of a fast-paced team.
• Knowledge of community resources and social service systems for referrals.
• Experience with vulnerable populations in shelter or similar settings preferred.
• Calm, compassionate approach with crisis de-escalation skills.
• Proficient with Microsoft Office and electronic record-keeping systems.
• Valid driver's license and clean driving record.
• Fingerprint/Card checks may be required for Phoenix programs.

Responsibilities

• Build trusting, respectful relationships with guests.
• Listen to guest needs and help identify next steps.
• Gather intake information (demographics, needs).
• Help guests create simple service or case plans.
• Connect guests to housing, benefits, health care, and behavioral health services.
• Monitor goals and referrals to encourage progress.
• Advocate for guest needs with staff and partners.
• Document notes and reports in the electronic systems.
