Job summary

The Society of St. Vincent de Paul - Phoenix is seeking a Case Manager to support residents in shelter and transitional housing.

You will provide trauma-informed, strengths-based case management, helping residents access services, advocate for needs, and overcome barriers to permanent housing. Responsibilities include developing individualized plans, linking to resources, monitoring progress, and collaborating with staff to ensure timely documentation and effective outcomes.

Qualifications

• Bachelor’s degree in social work or related field preferred; HS diploma or GED required.
• 1+ year in Case Management or 2+ years in human services preferred.
• Trauma-informed and strengths-based approach to support.
• Strong communication, both written and verbal.

Responsibilities

• Carry a caseload of residents, meeting weekly with each resident.
• Develop assessments and individualized service plans to identify needs and barriers.
• Monitor progress toward stabilization, income, and housing goals.
• Maintain records in data systems within 24 hours of interaction.
• Coordinate intervention with staff and security; use de-escalation when needed.
• Assist residents in accessing services, housing searches, and move-in logistics.
• Attend staff meetings, case conferences, trainings, and agency events.
• Support volunteers and engagement opportunities.
• Other duties as assigned by program management.
