Job summary

Elite Home Health Care, Inc. is seeking a driven Community Outreach Representative to promote home care services in NYC and surrounding areas. You will develop strategic partnerships and referral networks with hospitals, clinics, and social services to grow census and awareness.

Travel daily within the downstate territory is required. The role emphasizes relationship-building, community events, and education about MLTC and Medicaid programs, with bilingual Spanish-English preferred.

Qualifications

• Minimum 1 year of sales, healthcare marketing, or community outreach experience.
• Knowledge of MLTC, Medicaid, and community-based healthcare services preferred.
• Strong existing relationships within NYC/downstate healthcare networks is highly desirable.
• Excellent communication, presentation, and interpersonal skills.
• Self-motivated, organized, and able to work independently.
• Willingness to coordinate and host community events and presentations.
• Bilingual in Spanish and English preferred.

Responsibilities

• Build, maintain, and strengthen relationships with hospitals, nursing homes, physician offices, rehabilitation facilities, adult day health programs, social workers, case managers, and community organizations.
• Develop and manage referral sources to drive census growth and increase community awareness of agency services.
• Promote agency services through community outreach efforts, presentations, networking events, support groups, health fairs, and community-based events.
• Conduct home visits, intake support, and personalized appointments as needed to assist with the enrollment process.
• Follow up promptly on referrals and leads received from families, caregivers, healthcare providers, and community partners.
• Maintain ongoing communication with referral sources to ensure strong partnerships and exceptional customer service.
• Submit daily activity and outreach reports to management.
• Represent the agency professionally within the community and identify opportunities for market growth.

Summary

Pay Range

The hiring range for this position is $55,000–$65,000 annually, plus commission. Various factors will determine final compensation such as a candidate’s years of relevant work experience, skills, certifications, and location.
