Job summary

Elite Home Health Care seeks a driven Community Outreach Representative to promote our home care services across NYC and surrounding areas. The role focuses on building strategic partnerships to generate high-quality referrals and census growth.

Ideal candidates have home care marketing or outreach experience, knowledge of MLTC/Medicaid, and the ability to build relationships within the downstate market. Bilingual Spanish and English is preferred.

Qualifications

• Minimum of 1 year of sales, healthcare marketing, or community outreach experience.
• Experience in home care or MLTC/Medicaid preferred.
• Strong relationships within NYC/downstate healthcare networks.
• Excellent communication, presentation, and interpersonal abilities.

Responsibilities

• Build and maintain relationships with hospitals, nursing homes, physician offices, and community orgs.
• Develop and manage referral sources to grow census and awareness.
• Promote services through outreach, events, health fairs, and presentations.
• Conduct home visits and enrollment support as needed.
• Follow up on referrals from families and providers.
• Maintain ongoing communication with referral sources for strong partnerships.
• Submit daily outreach reports to management.
• Represent the agency professionally in the community.

Summary

This roleis responsible fordeveloping andmaintainingstrategic community partnerships that generate quality referrals for home care services.We are looking for candidates with home care marketing or community outreach experience, knowledge of MLTC and Medicaid programs, and the ability to build strong referral relationships within the downstate New York market. Essential Duties & Responsibilities

• Build, maintain, and strengthen relationships with hospitals, nursing homes, physician offices, rehabilitation facilities, adult day health programs, social workers, case managers, and community organizations.
• Develop and manage referral sources to drive census growth and increase community awareness of agency services.
• Promote agency services through community outreach efforts, presentations, networking events, support groups, health fairs, and community-based events.
• Conduct home visits, intakesupport, and personalized appointments as needed to assist with the enrollment process.
• Follow up promptly on referrals and leads received from families, caregivers, healthcare providers, and community partners.
• Maintain ongoing communication with referral sources to ensure strong partnerships and exceptional customer service.
• Submit daily activity and outreach reports to management.
• Represent the agency professionally within the community and identify opportunities for market growth.

Qualifications & Skills

• Minimum of 1 year of sales, healthcare marketing, or community outreach experiencerequired; home care industry experiencestronglypreferred.
• Knowledge of MLTC, Medicaid, and community-based healthcare servicespreferred.
• Strong existing relationships within the NYC/downstate healthcare and community network are highly desirable.
• Excellent communication, presentation, and interpersonal skills.
• Self-motivated, organized, and goal-oriented with the ability to work independently.
• Ability to coordinate and host community events and presentations.
• Must have reliable transportation and the ability to travel daily throughout the assigned territory.
• Bilingual in Spanish and English preferred.

Pay Range

The hiring range for this position is $55,000–$65,000 annually, plus commission. Various factors will determine final compensation such as a candidate’s years of relevant work experience, skills, certifications, and location.
