Elite Home Health Care is seeking a driven Community Outreach Representative to grow our presence in NYC and the greater downstate area. This field-based role focuses on developing partnerships that generate quality referrals for home care services and improving community awareness of agency offerings.
Ideal candidates have home care marketing or outreach experience, knowledge of MLTC/Medicaid, and strong referral networks in the NYC/downstate market. Bilingual Spanish/English is preferred.

Home Care Agency isseekinga driven and relationship-focused Community Outreach Representative to promote and grow our presence within NYC and the greater surrounding area. This roleis responsible fordeveloping andmaintainingstrategic community partnerships that generate quality referrals for home care services.We are looking for candidates with home care marketing or community outreach experience, knowledge of MLTC and Medicaid programs, and the ability to build strong referral relationships within the downstate New York market. Build,maintain, and strengthen relationships with hospitals, nursing homes, physician offices, rehabilitation facilities, adult day health programs, social workers, case managers, and community organizations.
Develop and manage referral sources to drive census growth and increase community awareness of agency services.
Promote agency services through community outreach efforts, presentations, networking events, support groups, health fairs, and community-based events.
Conduct home visits,intakesupport, and personalized appointments as needed toassistwith the enrollment process.
Follow up promptly on referrals and leads received from families, caregivers, healthcare providers, and community partners.
Maintain ongoing communication with referral sources to ensure strong partnerships and exceptional customer service.
Submit daily activity and outreach reports to management.
Represent the agency professionally within the community andidentifyopportunities for market growth.
Minimum of 1 year of sales, healthcare marketing, or community outreach experiencerequired; home care industry experiencestronglypreferred.
Knowledge of MLTC, Medicaid, and community-based healthcare servicespreferred.
Strong existing relationships within the NYC/downstate healthcare and community network are highly desirable.
Excellent communication, presentation, and interpersonal skills.
Self-motivated, organized, and goal-oriented with the ability to work independently.
Ability to coordinate and host community events and presentations.
Must have reliable transportation and the ability to travel daily throughout the assigned territory.
Bilingual in Spanish and English preferred.
Field-based / on-the-road position
Full-time, 8-hour shift
The hiring range for this position is $55,000–$60,000 annually, plus commission. Various factors will determine final compensation such as a candidate’s years of relevant work experience, skills, certifications, and location.
